Memorial Birth Certificate – To Remember That Special Moment

In November 2005 Higashikawa began issuing memorial birth certificates as a reminder that special moment of the birth of a child.
Nothing can be compared to the feeling of having a child, and the birth of a child is a moment full of excitement and joy. A child’s life starts with the birth certificate, and when it is viewed as a reminder of parents’ caring throughout their child’s life, we may say that it has the power to make family ties even stronger.
Memorial birth certificates issued by Higashikawa are souvenirs that remind one of that important moment of a child’s birth.

How to Receive the Memorial Birth Certificate

The memorial birth certificate (Shin-shussho todoke) is a souvenir from Higashikawa and it must be received in person. The birth certificate needs to be filled out, sealed, and submitted to Higashikawa Town Hall in person (not by mail).
There are two ways to receive the birth certificate:

  1. 1. Receive the birth certificate in person in Higashikawa Town Hall

    ※ Weekdays: 8:30 to 17:15 (except for New Year’s holidays)

  2. 2. Submit the birth certificate at Higashikawa Town Hall and receive it at a later date

    ※ Weekdays: 8:30 to 17:15. After examination, the birth certificate will be sealed with a date-stamp. It will be returned to you at a later date.
    ※ Weekends, holidays, after-hours: the birth certificate will be collected by a security guard. After examination, it will be returned to you at a later date.

Birth Registration

Necessary documents
  • 1) Birth certificate (Shin-shussho todoke)
  • 2) Seal (Inkan; the same one which was used to stamp the birth certificate)
  • 3) Maternity passbook (Boshi techo)
  • 4) Health insurance card (Kenko hoken-sho)

Weekday notification time period

1. Advance preparation
  1. Go to Higashikawa Town Hall and obtain the birth certificate. Staff will explain the contents of the certificate that require special attention.

    ※ Birth certificates can be obtained on weekdays from 8:30 to 17:15 (except for New Year’s holidays).

  2. Inform your hospital if you want to use the Higashikawa Memorial Certificate.

    ※ If not, you will receive a normal Birth Certificate according to the normal procedure.

2. Filling out the birth certificate
  1. Receive the certificate from your hospital.
  2. Fill out the birth certificate, sign and seal it.
3. Submitting the birth certificate
  1. Submit the birth certificate at Higashikawa Town Hall, including the following documents:
    • 1) Birth certificate
    • 2) Seal (Inkan; the same one which was used to stamp the birth certificate)
    • 3) Maternity passbook
    • 4) Health insurance card

    ※ Documents can be submitted on weekdays from 8:30 to 17:15 (except for New Year’s holidays).

  2. Other procedures (for Higashikawa residents only)
    • 1) Application for Subsidy of Medical Costs for Infants
    • 2) Application for Child Allowance
    • 3) Application for Lump-sum Maternity Allowance of National Health Insurance
4. Receiving the birth certificate
  • Submitted documents will be examined (it takes about 40 minutes).
  • If you have submitted all the necessary documents and filled out the birth certificate correctly, it will be sealed with a date-stamp and returned to you.

Weekend, holiday, after-hour notification time period

The birth certificate will be collected by a security guard. After examination, it will be returned to you at a later date.

3. Submitting the birth certificate
  1. Submit the family registry envelope

    ※ Fill in the registration date, name, address, phone number and desired receiving method.

  2. Include the following documents:
    • A) Birth certificate

    ※ Seal the envelope and stamp overlapping the closure flap.

4. Receiving the birth certificate
  • Submitted documents will be examined in a few days.
  • If you have submitted all the necessary documents and filled out the birth certificate correctly, it will be sealed with a date-stamp and returned to you at a later date according to the indicated receiving method.

    ※ Upon receiving the birth certificate, bring the following documents with you:

    • 1) Seal (Inkan; the same one which was used to stamp the birth certificate)
    • 2) Maternity passbook
    • 3) Health insurance card
  • Other procedures (for Higashikawa residents only) ※ (On weekdays only)
    • 1) Application for Subsidy of Medical Costs for Infants
    • 2) Application for Child Allowance
    • 3) Application for Lump-sum Maternity Allowance of National Health Insurance

Birth Certificate Handling Procedures

  • Information from the submitted birth certificate is copied to the cover sheet (memorial birth certificate), sealed with a date-stamp, and returned to you.
  • Paste your and your child’s photo on the message sheet, write a personal message to your child, etc., and put it in the sheet pocket.
  • You can also add other photos (your child’s first foot-prints, baby’s ultrasound picture, etc.) and make it your “time capsule”.
  • You can open your “time capsule” and take a look at it any time you want. But you can also try to keep it unopened, the longer the better.
  • You can save this “time capsule” for celebrating the important moments of your child’s life, such as when your child is getting married.

Submission of a Birth Certificate

Instructions for submitting a birth certificate

Before submission

Fill out the birth certificate and submit it
to Higashikawa Town Hall.
(Do not separate the sheets)

After submission

  • After the birth certificate has been submitted both sheets are separated and stamped with the commemorative seal. Both sheets are returned to you.

  • Paste your and your child’s photo on the message sheet, write your child’s name on it, and also write a personal message to your child.

  • Pockets can be used as a “time capsule” – for your message sheet, first photos of your child, etc. It will become a nice souvenir later on.

  • Peel the double-sided tape from both sheets and seal the pockets.

  • You can open your “time capsule” and take a look at it any time you want. But you can also try to keep it unopened, for as long as you please.

  • Enjoy the day you decide to open your “time capsule”!

The Memorial Birth Certificate, like the Memorial Marriage Certificate, is a part of the “people-friendly service”, adopted from the Fuji TV Program “New Design Paradise” (Kanto area only). The project was formed in cooperation with Amuse Inc. (TV production), and designed by the world-famous Yasushi Fujimoto, the art director of the design studio “Cap”.

Yasushi Fujimoto
“Cap”, Art Director

In 1983, Yasushi Fujimoto established his own art studio “Cap”. He has worked for over 100 magazines including Marie Claire Japan, Ryuko Tsushin, Olive, Studio Voice, Vogue Nippon, GQ Japan, Ginza, Brutus, Casabrutus, Real Simple, and Tokion. Fujimoto has also directed advertising projects for clients including Louis Vuitton, Omotesando Hills and several apparel brands.
In 1996, Fujimoto opened “Gallery Rocket”. The gallery was closed, but it reopened in Minami-Aoyama in Summer 2007. In March 2004, he opened an exhibition at the Ginza Graphic Gallery devoted to exhibiting and promoting graphic design. In the same year, he released a book called “Cap Magazine Designer’s Collective” (Zasshi wo Design Suru Shudan Cap). In 2006, he released a book called “The Secret Sense of Japanese Magazine Design” (Zasshi wo Design Suru Hito to Genba to Sensu no Himitsu).
